Occupational Permit

An individual Mayor’s Permit is required for workers or employees, whether temporary or permanent, working within the jurisdiction of the City.

Requirements

• • Medical Laboratory Examination Results
    • Chest X-Ray (1 Year Validity)
    • Urinalysis (6 Months Validity)
    • Fecalysis (6 Months Validity)

• • Community Tax Certificate (Cedula - Current Year)
  • 1 pc. 1×1 I.D. Picture
  • Other requirement:
    • Parents/Guardian’s Consent with photocopy of their ID (For 17 years old and below)
